What You'Ll Do
- Provides social work services in accordance with the physician's plan of treatment.
- Works with the interdisciplinary team directly, or through coordination of team members, to provide appropriate assessment and support patients and families.
- Provides assessment for normal and complicated grief and provides bereavement counseling as appropriate.
- Serves as a liaison with other community resources.
Job Requirements
- Master's or Bachelor's level social work degree from a school of social work accredited or approved by the Council of Social Work Education.
- Currently licensed to practice as a social worker in the state of SC.
- Minimum of one-year Hospice experience preferred.
- Valid Driver's License.
