Overview
The Bereavement Coordinator is responsible for overseeing all aspects of the Carle Hospice Bereavement Program. Bereavement services are provided for a one-year period following the patient's death.

Specialized Knowledge and Skills Requirements
Possesses an understanding of family dynamics and hospice care. Able to work as a member of an interdisciplinary team. Skilled in interview techniques and able to accurately assess others' needs and concerns. Able to work with available computer software programs. Detail oriented, and able to manage multiple tasks. Ability to network and organize community events. Ability to be flexible and adapt to rapid change.
Responsibilities: Essential Functions
Ensures that individualized grief risk assessments are completed for all significant family members and caregivers of hospice patients.
Develops a bereavement plan of care to address family member/care giver needs.
Provides routine bereavement services to family and caregivers regardless of risk factors.
Refers individuals identified as high risk for complicated bereavement to appropriate community based support resources.
Implements and continuously evaluates the effectiveness of the bereavement plan.
Provides ongoing orientation, training, support, development and supervision of trained bereavement volunteers.
Clearly documents bereavement plan, interventions, and measure of effectiveness in the bereavement record. (Completes personal documentation and ensures that other team members and volunteers contribute documentation which completes the bereavement record
Oversees all Carle Hospice Bereavement Services which may include: a. limited individual counseling b. grief support groups c. patient, family and caregiver support visits d. availability of telephone support e. scheduled mailings (personal, educational and informational) f. memorial services g. internal in-service programs h. external educational offerings I. referral to community services.
Measures, analyzes and reports to leadership on bereavement program effectiveness
Make bereavement services available to the family and other individuals in the bereavement plan of care for up to one year, extending to residents of SNF/NF or ICF when appropriate.
Department Specific Job Function Perform initial Spiritual Assessments for patient
Develop a Spiritual Care Plan for new admission patient's requesting service
Provide spiritual Support for patients & families per care plan
Provide spiritual education & resources to patients, families & staff
Participate in scheduled IDT meetings to share spiritual needs for patients, families & staff

Compensation and Benefits
The compensation range for this position is $20.18per hour - $32.69per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
