Are you a compassionate, experienced RN who loves making personal connections and helping patients understand their options at a vulnerable time? As a Hospice Admissions Nurse, you'll be the face of our care-helping families say "yes" to compassionate end-of-life support when it matters most.
What You'll Do as a Hospice RN Admissions Nurse:
- Report directly to the Admissions Manager and serve as a key ambassador for our hospice services• Visit patients and families in their homes, hospitals, or facilities to assess eligibility for hospice care• Explain hospice philosophy and services clearly, compassionately, and with clinical confidence• Guide patients and their loved ones through the admissions process, ensuring a smooth and supportive start to care• Collaborate with referring physicians, case managers, and hospital or facility staff to coordinate transitions of care• Use the nursing process to assess patient needs, support care planning, and document clinical findings• Build trust and provide emotional reassurance to patients and families as they make this important decision• Accurately complete required documentation and ensure compliance with state and federal regulations• Work closely with the clinical and interdisciplinary team to ensure timely admissions and quality care handoff
About You
Qualifications What You'Ll Bring
- Active RN license in the state of employment (or eligible to obtain)• 1+ year of general nursing experience• A hospice heart: compassionate, empathetic, and deeply committed to patient-centered care• Strong assessment, communication, and interpersonal skills• Comfortable visiting patients in hospitals, skilled nursing facilities, or home settings• Valid driver's license, auto insurance, and reliable transportation• Current CPR certification
Preferred Experience (Not Required)
- 2+ years of hospice, home health, healthcare admissions, or medical sales• Bachelor's Degree and/or Certified Hospice and Palliative Nurse (CHPN) certification• Background in case management, care coordination, or healthcare outreach• Experience with electronic medical records and compliance documentation
