This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services. **Summary of Responsibilities** - Provide a full range of Speech Language Pathology Services as ordered by the patient's physician. - Participate with all other home care personnel in patient care planning. - Instruct patients and family members in home programs. - Comply with Medicare documentation requirements. - Help develop the plan of care and revise as necessary. - Advise and consult with the family and other Agency personnel. - Participate in in-service programs. - Appropriately utilize ICD-10 codes. - Participate in QAPI activities as assigned. - Any other duties as required by the Director of Clinical Services. - Master’s or doctoral degree in Speech Language Pathology - Currently licensed to practice as a Speech Language Pathologist in Washington State - Two (2) years’ experience, preferred - Home Health experience, preferred - Ability to travel within Pierce and/or Thurston County, require **About Careage Home Health** Careage Home Health provides at home care for patients that reside within King, Pierce, and Thurston Counties in Washington State.
