## Description

## Social Worker Medical MSW

Medical Social Worker- MSW

**Schedule/Status: 24 Hours**  
**Department:** Hospice/Home Health  
**Service Area: Windham/Gorham/Standish**, Maine  
**Role:** Medical Social Worker - MSW  
**Work Setting:** Community Care Facilities and Homes

**What You’ll Do:**

- Assesses and identify the client’s holistic health care needs/concerns as part of a family and community.
- Assesses social/emotional factors that are related to the patient’s illness, need for care, response to treatment, and adjustment to care.
- Develops a plan of care under physician orders with realistic/measurable outcomes in collaboration with the client/family and health care team providers according to Organization protocols.
- Implement patient care plan activities based on treatment goals towards independent living for the patient/family.
- Serve as resource for staff education & training, participate in case conferences as needed.

**What You’ll Bring:**

- Master’s degree from a school of social work accredited by the council on social work education.
- Maine Board of Social Work Licensure- MSW or LCSW required
- Current CPR card required.
- One or more years of experience as a master’s level social worker in a health care setting.
- A valid Drivers License and reliable car with insurance.
