## Description

## Social Worker Medical MSW

**Schedule/Status: Full-Time, Exempt (Salary)**  
**Department:** Home Health  
**Service Area: Scarborough/Portland**, Maine  
**Role:** Medical Social Worker  
**Work Setting:** Community Care Facilities and Homes

Andwell Health Partners is seeking a compassionate and dedicated **Medical Social Worker** to join our healthcare team. In this role, you will provide psychosocial support, counseling, care coordination, and community resource referrals to patients and families navigating complex medical conditions.

**Key Responsibilities:**

- Assess patients’ social, emotional, and environmental needs.
- Develop and implement individualized care plans.
- Provide counseling, crisis intervention, and patient advocacy.
- Collaborate with physicians, nurses, and interdisciplinary team members.
- Connect patients and families with community resources and support services.
- Maintain accurate documentation in compliance with organizational and regulatory guidelines.

**Qualifications:**

- Degree in Social Work (or related field) from an accredited program.
- Current social work licensure or eligibility for licensure, as required. LSX Minimum.
- Experience in healthcare, home health, hospice, or community-based services preferred.
- Strong communication, assessment, and care coordination skills.
