You'll: - Manage a patient caseload and coordinate all aspects of care planning and delivery - Complete comprehensive assessments addressing physical, emotional, social, and spiritual needs - Collaborate closely with physicians and the interdisciplinary care team to provide exceptional patient-centered care - Conduct routine visits and adjust care plans as patient needs evolve - Guide and support patients and families through the hospice experience - Maintain accurate and timely clinical documentation - Supervise Home Health Aides and ensure quality care delivery in accordance with regulatory guidelines - Provide compassionate end-of-life care and family support **Qualifications** - Graduate of an accredited Registered Nursing program - Current RN license in the state of practice - Current CPR and First Aid certification - Knowledge of quality assessment and assurance procedures - Valid driver's license and proof of insurance - Excellent communication, assessment, and organizational skills - Passion for delivering compassionate, patient-centered hospice care **Join a Team with Purpose** At Hospice of Virginia, you're more than a nurse.
