## Description

**Full-Time Home Health Registered Nurse**  

**Clarksville, TN and Surrounding areas!**

**Key Responsibilities**

- Perform patient assessments, admissions, and ongoing nursing evaluations

- Develop and update individualized plans of care

- Coordinate care with physicians and the interdisciplinary healthcare team

- Provide skilled therapeutic, preventative, and rehabilitative nursing care

- Monitor patient progress and communicate changes in condition

- Educate patients and caregivers on disease management, medications, and treatment plans

- Maintain accurate and timely clinical documentation

- Deliver compassionate, patient-centered care in the home setting

**Qualifications**

- Graduate of an accredited nursing program

- Current Registered Nurse (RN) license in the state of Tennessee

- Minimum of one (1) year of clinical nursing experience (ICU, Med-Surg, or Rehabilitation preferred)

- Home Health experience preferred, but not required

- Current CPR certification

- Valid driver’s license, reliable transportation, and proof of automobile insurance
