## Description

**Klarus Home Health** is seeking a dedicated and professional **Physical Therapist** to join our team serving our patients currently enrolled in our **home health services** in **San Antonio, TX** and surrounding areas.

This role involves providing coordinated care to patients/clients of all age groups, planning and implementing individualized care plans to restore or maintain patient well-being. The Physical Therapist will plan, organize, and conduct physical therapy treatment programs based on medical referrals and their evaluations, ensuring effective coordination of care.

**Key Responsibilities:**

- Develop and implement individualized physical therapy treatment plans.
- Evaluate and document patient/client progress, adjusting treatments as necessary.
- Coordinate care with other healthcare professionals to ensure comprehensive treatment.
- Educate patients/clients and their families on treatment plans, home exercises, and preventative care.
- Maintain accurate and timely patient records in compliance with company and regulatory standards.
- Utilize durable medical equipment as needed and provide training to patients/clients.
- Ensure patient safety and comfort during therapy sessions.
- Participate in continuous professional development and stay updated with the latest therapy practices.

**Job Qualifications:**

- **Education:** Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program.
- **Licensure:** Current State License or a multi-state license issued by a state member of the Physical Therapist Licensure Compact (PTLC) if practicing in a state member of the PTLC, or meets one of the requirements listed in 42 CFR §484.115(h).
- **Experience:** At least one year of experience as a Physical Therapist in an acute care, rehabilitation setting, or home health preferred.
- **Skills:** Proficiency in therapy skills as defined by accepted standards of practice. Strong interpersonal skills. Current CPR certification. Knowledge of durable medical equipment.
- **Transportation:** Reliable transportation, a current Driver’s License, and valid and current auto liability insurance.

**Environmental and Working Conditions:**

- Work in healthcare facilities or patients’/clients’ homes in various conditions.
- Possible exposure to blood, bodily fluids, and infectious diseases.
- Ability to work a flexible schedule and travel locally.
- Some exposure to unpleasant weather conditions.

**Physical and Mental Effort:**

- Ability to perform heavy lifting, bending, pulling, pushing, and prolonged standing and walking.
- Must be able to address patient/client and family psychosocial needs effectively.
- Requires good hand-eye coordination and manual dexterity.
