### Position Summary

FYZICAL Therapy & Balance Centers in **Lake City, FL** is seeking a compassionate and motivated **Physical Therapist** to join our outpatient team.

In this role, you will evaluate and treat patients with a variety of orthopedic, neurological, balance, and mobility-related conditions while developing individualized treatment plans based on each patient's needs and goals.

Whether you're an experienced Physical Therapist or a new graduate beginning your career, you'll have access to mentorship, continuing education, specialty training, and opportunities to grow professionally while making a meaningful impact on the patients you serve.

### Key Responsibilities

- Evaluate patients and develop individualized plans of care based on clinical findings and patient goals
- Provide evidence-based physical therapy treatments and interventions
- Treat a diverse outpatient patient population with a variety of diagnoses and functional needs
- Monitor patient progress and modify treatment plans as appropriate
- Educate patients and caregivers regarding treatment plans, therapeutic exercises, and strategies to support continued progress
- Maintain accurate and timely clinical documentation
- Communicate and collaborate with physicians, healthcare providers, and members of the clinical team
- Provide a positive, patient-centered experience throughout the course of care
- Follow applicable professional, ethical, regulatory, and clinic standards

### Qualifications

- Graduate of an accredited Physical Therapy program
- Current **Florida Physical Therapist license or eligibility for Florida licensure**
- Strong communication and interpersonal skills
- Commitment to providing high-quality, patient-centered care
- Ability to work collaboratively as part of a clinical team
- Desire to continue learning and developing professionally
- Outpatient experience is a plus but not required
- **New graduates are encouraged to apply**
