## Description

**Full-Time Home Health Physical Therapist – Camden, TN**

Looking for a Physical Therapist position that offers competitive compensation, greater schedule flexibility, and meaningful one-on-one patient care? Tennessee Quality Care is hiring a **Full-time Physical Therapist** to serve patients in **Camden and the surrounding counties**.

**Key Responsibilities**

- Evaluate patients and develop individualized treatment plans based on physician orders
- Provide skilled physical therapy services in accordance with the physician’s plan of care and state regulations
- Help patients improve mobility, strength, balance, and functional independence
- Educate patients, families, and caregivers on treatment plans, home exercise programs, and safety techniques
- Recommend adaptive equipment and home modifications to promote safety and independence
- Collaborate with physicians and the interdisciplinary care team to achieve optimal patient outcomes
- Complete accurate and timely clinical documentation and care plan updates
- Supervise Physical Therapist Assistants (PTAs) in accordance with company policy and state regulations
- Encourage and motivate patients throughout their rehabilitation journey

**Qualifications**

- Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA)
- Current Physical Therapist (PT) license in the state of Tennessee
- Home Health experience preferred, but not required
- Two (2) years of experience in an acute care or similar clinical setting preferred
- Current CPR and First Aid certification
- Valid driver’s license, reliable transportation, and proof of automobile insurance
