Innovations in care delivery include trauma-informed and integrative care, HUB sites where patients can access co-located and integrated behavioral health care, EmPATH (Emergency Psychiatric Assessment, Treatment, and Healing) units, and Recovery Services. **Location:** PHP and IOP services at our Riverside location. **Schedule:** Mon-Fri 8am-4:30pm **Responsibilities:**  As a **Licensed Programmatic Care Therapist**, you will determine clinical interventions, facilitate group therapy and provide education for patients and their families. You will facilitate treatment planning and manage each case. You're responsible for development of the transition/discharge plan. You will function as an interdisciplinary team member. In addition to the above, you will be responsible for the following:  - Demonstrates clinical competence in the treatment planning process - Demonstrates clinical competence to assess individual patient needs - Demonstrates clinical competency to partner with multi-disciplinary team in the treatment planning process. - Demonstrates competence in developing a clinically relevant transition/discharge plan. - Demonstrates clinical competence in conducting group therapy, family therapy and psycho-education groups - Demonstrates the ability to advance evidence based practice by utilizing standards, guidelines and pathways for care delivery - Communicates effectively and respectfully with others, including interdisciplinary team and all other contacts **Qualifications:**  **Required** **Education** - Masters in Social Work and/or Psychology **Experience** - At least two years’ experience    BLS required prior to starting **License/Certification/Registration** - One of the following current Minnesota licenses * Licensed Independent Clinical Social Worker (LICSW) * Licensed Psychologist * Licensed Professional Clinical Counselor (LPCC) * Licensed Marriage and Family Therapist (LMFT) - BLS required prior to starting **Preferred** **Experience**  - Two years’ supervised therapy experience in mental health including assessment, individual, group and family therapy. **License/Certification/Registration** - Ability to supervise **Additional Requirements** (must be obtained or completed within a period of time):   For *outpatient units*, De-escalation and Self-Defense training and retraining as required. **Benefit Overview** Fairview offers a generous benefits package, including but not limited to medical, dental, vision, PTO/vacation and Safe and Sick Time, tuition reimbursement, retirement and more!
