The LPN assists with patient assessments, treatments, medication administration, diagnostic testing, patient education, and care coordination to support quality healthcare delivery. Supports and follows the vision, mission, goals, and objectives of Family Healthcare of Hagerstown. **Responsibilities and Duties** - Assist with patient assessments, screenings, and collection of clinical information. - Administer medications, immunizations, and treatments as ordered by providers and within scope of practice. - Perform clinical procedures and point-of-care testing, including but not limited to A1C testing, EKGs, urinalysis, and other diagnostic services as assigned. - Prepare patients for examinations, procedures, and treatments. - Provide patient education regarding medications, testing, treatments, and follow-up care. - Accurately document patient information, assessments, interventions, and communications within the Electronic Health Record (EHR). - Review and communicate test results and provider instructions to patients as directed. - Assist providers with examinations, procedures, and patient care activities. - Coordinate patient care activities and facilitate appropriate follow-up appointments and referrals. - Maintain exam rooms, clinical equipment, and supplies to support efficient patient care. - Adhere to infection control, safety, confidentiality, and regulatory requirements. - Foster positive communication and professional relationships with patients, families, providers, staff members, and community partners. - Participate in quality improvement initiatives, staff education, and professional development activities. - Perform other duties as assigned. **Benefits** Generous benefits package including accrued paid time off, seven (7) paid holidays, and one Anniversary Award after the first year of employment. Voluntary benefits include cancer, hospital indemnity, accident coverage, and more. **Position Qualifications** **Education** - Graduate of an approved Licensed Practical Nursing (LPN) program required. **Experience** - Pediatric and Family Practice experience preferred; new graduates will be considered. - Experience working in a medical office, primary care, community health, or ambulatory care setting preferred. - Proficient typing skills and ability to efficiently enter information into the Electronic Health Record (EHR). - Basic computer skills required, including the ability to navigate software applications, manage electronic documentation, and utilize healthcare technology. **Licensure/Certifications** - Current Licensed Practical Nurse (LPN) license in the State of Maryland or compact state required. - Current Basic Life Support (BLS) certification required. **Other Requirements** - Strong communication and interpersonal skills. - Ability to work effectively in a team-based healthcare environment. - Commitment to delivering compassionate, patient-centered care. - Ability to maintain confidentiality and professionalism in all interactions. **Physical and Mental Effort** **Physical:** Moderate physical effort is required.
