MACV provides comprehensive programs in the areas of housing, employment, legal, and other services to support Veterans and their families throughout Minnesota who are homeless or at risk of homelessness. MACV operates a budget of over $15 million annually and has a team of approximately 120 employees throughout Minnesota. The organization has office locations throughout the state, with administrative functions centralized within a headquarters office based in St. Paul. MACV offers a competitive benefits package that includes healthcare coverage, dental, vision, life, STD and LTD insurance, generous paid time off, retirement benefits, 12 holidays per year, paid parental leave, paid family and medical leave, technology and wellness reimbursement programs, tuition reimbursement, continued professional development support, and more. **Position Overview** The JIV Case Manager provides housing-focused case management and reentry support to Veterans whose active involvement with the criminal justice system creates a barrier to obtaining or maintaining stable housing. The position works with Veterans before and after release from correctional facilities and with Veterans in the community who have active justice-related housing barriers. This position will utilize a network of internal and external services to assist justice-involved Veterans in addressing housing and reentry barriers, maintaining housing stability, successfully transitioning back into the community, and working toward long-term success. Services are structured, goal-focused, and time-limited. The Case Manager will coordinate with MACV housing teams, correctional agencies, VA programs, Veterans Treatment Courts, County Veterans Service Offices, and community partners. This role is essential in providing consistent, supportive services that promote stability and reduce the risk of recidivism. **Work Environment** The JIV Case Manager works in offices, correctional facilities, court, and community-based settings throughout Minnesota. The position requires regular statewide travel to meet with Veterans, support pre-and post-release planning, participate in court and facility-based activities, and coordinate with MACV staff and community partners. Partial hybrid office work is permitted. **Primary Duties and Responsibilities** · Conduct regular in-person outreach with state, federal, and county correctional facilities to identify veterans at risk of homelessness upon release. · Collaborate with facility staff, MACV programs, and community partners to develop individualized reentry plans. · Complete client intakes, including documentation, screening, assessments, and program orientation. • Identify justice-related barriers affecting housing and develop individualized housing and reentry goals with the Veteran. • Coordinate pre-release and community reentry planning with correctional staff, probation/parole, VA programs, MACV teams, and other authorized partners. • Assist Veterans with identification, eligibility documents, housing applications, benefits referrals, and connections to appropriate housing and community resources. • Maintain regular contact with Veterans and provide structured support toward agreed-upon housing and reentry goals. • Coordinate referrals and transfers to MACV regional housing programs or outside providers when ongoing services are needed. • Participate in correctional transition fairs, Veterans Treatment Courts, case coordination meetings, outreach activities, and other JIV-related events. • Maintain timely and accurate case notes, service records, referrals, releases of information, and required documentation in Salesforce and other designated systems. • Protect confidential information and share information only when properly authorized and necessary for service coordination. • Communicate significant barriers, safety concerns, disengagement, or complex cases to the JIV Program Director. · Maintain a post-release caseload for Veterans with the highest barriers releasing from federal, state, and county correctional facilities.
