Responsibilities include: - Conducting pre-admission evaluations to determine eligibility and appropriateness for hospice services - Educating patients and families about hospice care and available services - Facilitating the admission process and obtaining informed consent - Completing comprehensive nursing assessments - Developing initial plans of care focused on comfort, dignity, and quality of life - Collaborating with the interdisciplinary team to ensure exceptional patient-centered care **Qualifications** - Graduate of an accredited Registered Nursing program - Current RN license in the state of Virginia - Current CPR certification - Hospice experience preferred (1+ year) - Strong clinical judgment and ability to work independently - Excellent communication and interpersonal skills - Ability to build trusting relationships with patients, families, and healthcare professionals - Knowledge of quality assessment and assurance practices **Join a Team That Cares** At Hospice of Virginia, we believe our employees are our greatest asset.
