### Role Summary

As a mid-level Physical Therapist supporting Buckeye and surrounding areas you will help home health patients regain mobility, improve function, and maintain independence through skilled, individualized care. This role exists to deliver safe, effective rehabilitation in the home setting while coordinating closely with the broader care team.  

Key responsibilities include completing OASIS start-of-care and follow-up evaluations; reviewing physician orders and medical history; establishing measurable goals and tailored plans of care; providing therapeutic exercise, manual techniques, and adaptive equipment training; educating patients/caregivers on home programs, body mechanics, and fall prevention; tracking outcomes and updating plans as needed; communicating progress with physicians and clinicians; and completing timely, accurate documentation in compliance with HIPAA and agency standards.  

Required qualifications/skills:  
- Active Arizona Physical Therapist license  
- Home health experience preferred  
- Strong assessment, communication, and independent caseload management skills  
- Ability to travel to patient homes within assigned territory

### Typical Workday

Your Monday–Friday rhythm balances focused in-home visits with structured planning time. You start by reviewing your schedule and patient notes, then head out for one-on-one sessions where you build rapport, set expectations with caregivers, and keep safety top of mind in each home environment. Between visits, you connect briefly with the clinical team to confirm updates and ensure continuity of care. You’ll reserve dedicated blocks to finalize documentation, reconcile orders, and prepare for upcoming evaluations so your caseload stays organized and compliant. The day ends with a clear plan for the next set of visits and any follow-ups needed to keep care moving forward.
