## Description

Elevate your career with a team that truly cares. The Certified Nursing Assistant is specifically trained, competent and performs assigned functions of personal care to the patient in their residence under the direction, instruction, and supervision of the registered nurse (RN).

You **must** have **CNA experience in the home health setting**. You must have **reliable transportation**. This is a great opportunity to join an established agency celebrating 40 years in the valley.

Responsibilities:

•Assist guests/patients during admissions, transfers, and discharges. Check each guest/patients routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes and the care plan.

•Review care plans daily to determine if changes in the guests/patient's daily care routine have been made on the care plan
