The program features the use of the Solution Based Casework model to partner with families in identifying the concerns and developing a service plan to address the concerns. **Essential Functions and Primary Responsibilities:** - Direct Services to clients via casework, individual, family and group counseling as indicated by individual case assessments. - Assist in Intake and Crisis Intervention - Conduct home visits and attend client related meetings as needed. - Develop client assessments and service plans following the model outlined by Improved Outcomes for children, Family Team Conference and SBC. - Monitor and assist in service provision to all program clients as required by program mandates. - Maintain current case records and service plans (FASP) for families served utilizing Connections and PROMIS systems. - Participate in weekly supervision, case conferences and Family Team Conferences as directed by Supervisor and Program Director. - Provide parent education and other support services utilizing group format as determined by client need. - Participate in training appropriate to the development of enhanced Social Work skills as required by Children’s Services and HeartShare St Vincent’s Services. - Assist in day to day operations as required by the Program Director. - Performs other job-related duties as assigned, consistent with the scope, responsibilities, and level of the position, and in compliance with applicable laws, regulations, and organizational policies. - Bachelors **required** or Masters Degree **preferred** - Two years experience in Child Welfare or related Human Service field. **Benefits** What we offer once YOU join our growing family: At Heartshare we offer a comprehensive benefit package based on full-time/part-time status.
