## Description

**Direct Support Professional - Residential**

**Location:** Madison County

**Schedule:** Full-time with day, evening and weekend shifts available

**Salary**: $20.00

**Sign On Bonus-up to $1,000**

**Benefits:** Include medical, dental, vision, life insurance, 401k employer match, tuition assistance, PTO, and 12 days of paid Holidays **About ACHIEVE's Residence Program**: In the residential program, the most caring people are the trained staff who provide 24-hour supervision, care, and guidance to residents. They assist with personal care needs, cleaning, cooking, medications, and activities. The **Direct Support Professional - Residential** provides self-care training and assistance in daily living to individuals with developmental disabilities residing in a group home setting.

**RESPONSIBILITIES**

- Provides individualized care as outlined in the individuals Life Plan, or ISP.
- Assists Individuals in the primary activities of daily living (toileting, feeding, cooking, cleaning) as outlined in individual's ISP to promote independence.
- Maintains files and produces service notes and documentation on assigned caseload.
- Documents and submits reports to the program as outlined in the assigned department.
- Model appropriate social skills to assist the individual in developing and maintaining relationships in the home and in the community
- Assists Individuals in the development of natural supports within the community.
- Attends all established or assigned in-services and web-based training.
- Provides First Aid, CPR, and SCIP-R as needed.
- Uses behavior management techniques as directed in the individual behavior plans and/or medication monitoring plans.
- Give medications as prescribed by physician, per OPWDD regulations

- High school diploma or GED; Valid NYS driver's license and 12 months driving history. Six months to one-year related experience and or training preferred.
