## Description

Valley’s [Forensics](https://valleycares.com/treatment-and-services/for-adults/outpatient-treatment) program empowers individuals to build stability, strengthen independence, and move toward meaningful life goals. As a Case Manager I, you'll partner with clients to connect them with resources, navigate challenges, and develop the skills needed for long-term success. Through personalized support, advocacy, and evidence-based case management, you'll help clients make lasting progress on their path to recovery and well-being.

**Pay:** Dependent on Case Manager certification status (see below)

- **Uncertified** start at $17.50 - will obtain certification within the first 120 days and then pay will be increased
- **Certified** range starts at $18.75 - pay is calculated based on years of certified experience.

**Schedule****:** Monday-Friday | 9:00am-5:00pm

- **Retirement** Our 401(k) plan accepts both pre-tax and Roth (post-tax) contributions and includes a company match of up to 6% of your annual salary.

**Job Summary & Deliverables**

The Case Manager I is a certified position that provides targeted case management and psychosocial rehabilitative services as indicated by the care plan to support clients to meet their individualized goals. Certified Case Managers are responsible for utilizing evidence-based practices, meeting fiscal responsibilities and ensuring that regulatory compliance and organizational standards are met.

- Provides targeted case management services as described by the Medicaid manual
- Supports clients in following individualized care plans and identifying case management goals
- Completes documentation on services provided within Valley’s standards
- Meets productivity expectations as assigned by supervisor
- Observes and collects client urine samples as necessary
- Transports clients to appointments as necessary

**Licenses/Certification**

- Case Manager certification (post hire as needed - required within 120 days of start date)
- CPR certification (post hire as needed)
- Driving positions require a minimum age of 21 and a current driver’s license

**Preferred Qualifications**

- Previous social services or behavioral health experience
- Bachelor’s degree in Social Services, Psychology, or related field
