Travels within the community and may transport clients using a personal vehicle. Works with clients and families to provide case management services and support their needs. Participates in treatment team meetings to help ensure client goals and recommendations are followed. Maintains accurate and timely documentation in the Electronic Medical Record system. Assists with client and family assessments to identify strengths and needs, monitors progress toward treatment goals, and communicates updates to the primary therapist. **Division/Program Overview:**  - **Population Served:** Serving adults, 18 plus - **Facility & Setting:** Outpatient community-based services and support. Delivers client services throughout Fresno County within designated county limits.
