To learn more about our company, visit [www.myisaachealth.com](http://www.myisaachealth.com/) **About the Role** We’re looking for a Care Navigator to join our existing care team who has prior experience coordinating/navigating care for patients, caregivers, and their loved ones - bonus points if you speak Spanish and you’ve worked with a patient population who experience cognitive challenges.
