## Description

**Average hourly rate is $19.23 (including wage, incentives, bonuses, overtime, shift differential, etc.)**  

Do you love helping children and families thrive? **TFI Family Services** is hiring a **full-time Aftercare Case Manager** to join our team!

### **What You’ll Need**

- A **bachelor’s degree in human services**.
- A **valid driver’s license**.
- A willingness to learn—we’ll provide all the training you need!

### **A Day in the Life**

As an Aftercare Case Manager, you’ll support children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care. You’ll:

- Focus on **risk management** and help families maintain stability during the aftercare period.
- Provide **crisis intervention** and work with public and private agencies offering supportive services.
- Maintain accurate documentation and reports.
- Coordinate community and educational services for families.
- Meet with families in-person to support reunification and resolve any concerns.
